NeuLife Independent Functional Evaluation

This comprehensive evaluation, dependent on each circumstance, helps determine the extent of an individual’s injury, the causal relationship and expectations for future care and services. 

Signs and Symptoms Need Further Review and Observation

  • This service is available when a diagnosis may or may not be related to a specific injury and/or when the presentation of an individual is not reflective of the definitive signs and symptoms of the diagnosis. In some of these situations, physicians who have already evaluated an individual are unable to explain any number of symptoms including pain, emotional or behavioral changes, physical or cognitive deficits, and an in-depth evaluation is needed. Individuals being considered may participate for any length of time but the typical length of stay is between 14 and 28 days.

Projection of Long-Term Needs for Care, Level of Supervision and Related Services

  • This service is available to help determine the long-term needs and supports of individuals who have suffered a catastrophic injury. The determination of specific needs can include the intensity of care, strengths and abilities, level of supervision and assistance, physical function, equipment required, the effectiveness of current equipment and its utilization, community access abilities, transportation needs, cognitive and behavioral function, and much more, dependent on each individual being considered. Our multidisciplinary team will provide care and assistance and offered therapies along with teaching and retraining as recommended during our evaluation and under physician’s orders. The length of stay for individuals being considered for participation is entirely dependent on specific needs and overall presentation.

NeuLife provides the following:

  • Record/file reviews to determine clinical relevance or to make recommendations on-site Screening Evaluation (at the hospital, rehab, home, anywhere) preparation of a formal Proposed Plan including comprehensive per diem/all-inclusive rate (pre-admission) fast processing for emergency/urgent admissions admission coordination
  • Scheduled conferencing of our client in therapy or during daily therapeutic activities – you can observe our client remotely. Note: This must be scheduled with appropriate signed consents and client’s knowledge and, as appropriate, the client’s family’s knowledge of the conference.
  • Timely completion of workers’ compensation (DWC-25) and/or liability insurance forms and other required documentation
  • Facility and grounds video monitored 24/7/365
  • Full-time licensed professional staff including but not limited to registered nurses, licensed practical nurses, board certified behavior analyst, speech language pathologist, occupational therapist, physical therapist
  • Continuing Education courses are available at no cost in your office

It is the mission of NeuLife as an organization to provide a comprehensive program of neurologic rehabilitation to support and promote the improvement of the quality of life for our patients.

As with all NeuLife provisions and specialized services, clients and designated family members and/or legal guardians participate in the referral, assessment and admission process. All services provided and those offered to our clients are arranged and coordinated by our care coordinators and are all-inclusive in a comprehensive per diem specific to each client.
