Customizing Your Goal Plan
Every individual admitted to NeuLife is unique and treated as such from our first meeting during our pre-admission evaluation. Client and family participation into the psychosocial history of each client plays an important part in the preparation of each Client Goal Plan. NeuLife has developed a multi-faceted system that starts by combining clinical and medical recommendations related to each client’s injury or diagnosis with focus on desired personal goals each client would like to achieve. It is clear that willing participation increases when our clients realize NeuLife’s Goal Plan strikes this balance and works with a holistic approach, encompassing all areas important to each client. Having a good time and laughing are encouraged at NeuLife!
The NeuLife Client Goal Plan encompasses factors including indicators in the medical records such as the type and severity of the injury or the diagnosis, Glasgow Coma Score (GCS) at the scene and on arrival at the trauma center, days, weeks or months in coma and with respiratory support, other bodily injuries, length of time hospitalized along with each client’s life history. This Plan is utilized during the initial assessment where additional information is determined by members of NeuLife’s multidisciplinary team and progress is monitored, tracked and measured from this point forward. Each client, designated family members and/or legal guardian, as well as involved professionals, can participate and offer continuous feedback and encouragement during multidisciplinary team conferences or as individually scheduled by involved family members or professionals.
